Youth Program registration form *— Registration application for students of Lower Division & Upper Division (Grades 1~12)
Kindergarten Program registration form — Registration application for applicants aged 4-5
School Rules *— Rules during school attendance
School Rules Verification *— Acknowledge understanding of School Rules with signature
Photo Release form *— Grant permission for use of images
High School credit rules **— Acknowledge requirements for receiving high school credit
Emergency Contact form *— Contact info if you are unavailable
Liability Release form *— Acknowledge risk and accept responsibility
Parent Agreement form *— Affirm awareness of expectations and commitment to your student’s experience at Sakura Gakuen
